What is Northern Health Home Care Packages?

Northern Health Home Care Packages is an approved provider of Commonwealth funded Home Care Packages. A Home Care Package helps you live independently in your own home with a package of care, services and case management to meet your personal needs.

Northern Health has been supporting older people with complex health and social situations to remain living at home since 1988. We have a proud history of providing quality case management and services to improve older people’s quality of life and overall wellness at home.

What can I expect from the service?

We offer a comprehensive range of services and supports to maintain your social connections and independence at home. Some of our responsive and flexible services include:

  • Specialist case management from a qualified health professional
  • Personal support (e.g. showering and dressing assistance)
  • Health and therapy services (e.g. nursing, occupational therapy, physiotherapy, podiatry and massage therapy)
  • Domestic assistance (e.g. cooking, meal preparation, cleaning, shopping and errands)
  • Home maintenance (e.g. gardening and gutter cleaning)
  • Transport assistance (e.g. appointments and activities)
  • Respite and carer support
  • Social connectedness (e.g. companionship, assistive devices, social activities and groups).
  • Home modifications

How much does it cost?

Northern Health Home Care Packages do not charge a daily care fee and will not deduct interpreter costs from your individual package funds. Depending on your income, you may be required to pay an Income Tested Fee (ITC), which is independently worked out by Centrelink. We do not charge an exit fee. This is specified in each Consumer Agreement and is published on the My Aged Care website .

Our team:

Our case managers are all experienced health professionals with tertiary qualifications in nursing or allied health (e.g. social work, occupational therapy and psychology).  We are a large team consisting of case managers (some of whom are bi-lingual), administrative staff, two dedicated clinical coordinators, a program manager, on-call case management systems and an after-hours support service.

Service Overview

Northern Health Home Care Packages is an approved provider of Commonwealth funded Home Care Packages. Home Care Packages support older adults with complex care needs to live at home with a package of care, services and case management. Working in partnership with consumers and their families, an appointed case manager helps to identify the consumer’s goals and support needs to form an individualised care plan. This is used to develop a support package of coordinated tailored services using consumer’s individual home care package funds.
Services that can be provided in Northern Health Home Care Packages include, but are not limited to:

  • Case management and care coordination
  • Personal support (e.g. showering and dressing assistance)
  • Health and therapy services (e.g. nursing, occupational therapy, physiotherapy, podiatry and massage therapy)
  • Domestic assistance (e.g. cooking, meal preparation, cleaning, shopping and errands)
  • Home maintenance (e.g. gardening and gutter cleaning)
  • Transport assistance (e.g. appointments and activities)
  • Respite and carer support
  • Social connectedness (e.g. companionship, assistive devices, social activities and groups).
  • Home modifications (related to care needs)

Our case managers are all experienced health professionals with tertiary qualifications in nursing or allied health (eg Social work, occupational therapy and psychology). We are experts within the community aged care industry-with the necessary knowledge and skills to assist consumers with their ongoing support issues and health needs.

Inclusion Criteria:

Northern Health Home Care Packages are targeted to assist frailer older Australians to remain in their homes, that have been approved by ACAS for a Home care Package.

Typically this includes:

  • People above 65 years of age, and
  • Aboriginal and Torres Strait Islander people above 50 years of age.

There is no minimum age requirements, citizenship or residency restrictions on accessing a Northern Health Home Care Package.
Younger people with a disability, dementia or special care needs may be eligible when no other more appropriate option is available.

Exclusion criteria:

Home Care Packages are not designed to act as a crisis management tool. Nor are they intended for visitors to Australia or for people requiring temporary or short term care.

Our service targets consumers living in the municipalities of Banyule, Darebin, Moreland, Nillumbik and Whittlesea, and parts of Brimbank, Manningham and Moonee Valley.

How to Refer:

My Aged Care is the entry point for accessing aged care services. Once assessed by the Aged Care Assessment Service (ACAS), approved consumers will be centrally recorded with My Aged Care and notified when they have been assigned a package.

Consumers can then select Northern Health Home Care Packages to be their home care package provider by directly contacting our service.
