Community Care Services provide care coordination to support people to live at home safely and independently and remain active members of their community.

We care for people with assigned Home Care Packages (HCPs) through My Aged Care and Home and Community Care Program for Younger People (HACC PYP) services (people aged under 65 years or 50 years and under for Aboriginal and Torres Strait Islander people).

Services we provide include:

Community Care Services can support you to tailor care and services you need, including:

• Assistance with bathing, toileting, dressing and undressing
• Assistance with laundry, house cleaning, gardening
• Assistance with shopping, support to attend social activities and appointments
• Equipment to assist with mobility
• Access to nursing, allied health services
