You Can Now Apply for an Aged Care Assessment Online

A new online form is available on My Aged Care, allowing Australians looking for aged care services (or those acting on their behalf) to apply for an aged care assessment, as an alternative to calling the My Aged Care contact centre.

The first step in accessing aged care services in Australia, is to find out whether you are eligible to receive government-subsidised services.

Apply for an aged care assessment online

You can apply online to check your eligibility using the new online form. You will be asked a series of questions including:

  • If you’re requesting an aged care assessment for yourself or someone else
  • Your date of birth
  • The type of care you’re looking for: help at home or move into a residential aged care home
  • The daily activities you need help or aids to do: walking, taking a shower, getting dressed etc.

You will also be required to provide your Medicare information, and will have the opportunity to set up the details for a representative to act on your behalf if needed.

The online application for an aged care assessment will take approximately 15 to 20 minutes.

After submitting the form, an aged care assessor will contact you within three weeks to organise a face-to-face assessment, which will help to determine the level of care required depending on your individual needs.
