| Low Level Care to the Home - www.health.gov.au CACPs are individually planned and coordinated packages of community aged care services, designed to meet older people's daily care needs in the community. CACPs are targeted at frail older people living in the community who require management of services because of their complex care needs. These people would otherwise be eligible for at least low level residential care. |
