Private Case Management

Private Case Management is the coordination of home care services to ensure that all of your care and support needs are being taken care of.

Private Case Managers will work with you to develop an individually tailored care plan that suits your specific needs. They essentially act as brokers for direct home care providers.

A Private Case Manager can be used if you want to use a range of home care services or wish to pay for your own home care without assistance from the government.

Private Case Managers can support you and coordinate all of your home care needs by dealing with service providers as well as medical and allied health professionals on your behalf. You may need home care services from a range of different providers. If this is the case, Private Case Managers will take care of all the arrangements for you.

You can employ a private case manager for short or long-term periods of time. It’s all up to you.

Private Case Managers are often used to take pressure off family members who do not wish or have the time to organise or manage home care services.

Private Case Management is not subsidised by the government, so you must pay for all of the services you receive.

Hourly rates can differ between Private Case Management providers, so it is important to ask for a detailed breakdown of costs before signing an agreement. An invoice should be provided to you each month outlining all the care purchases the Private Case Manager has made on your behalf.

Private Case Management services do not require an ACAT/ACAS assessment.

If you are the recipient of a Government-funded Home Care Package, a Case Manager from your chosen home care service provider will be assigned to you.
